Contractor Badging Requirements

As a contractor at Pomona Valley Medical Center (PVHMC), you will need four items to receive a security badge.

  1. Background Check
    • 7 Year National Criminal History Check
    • Sex Offender Clearance Check
    • Name and Address Verification Check
    • Social Security Number (SSN) Verification
  2. Identification
    • Copy of Identification Card or Driver’s License
  3. Immunizations
    • TB Screening
    • Flu Vaccination or Declination Form
    • MMR (Measles, Mumps, Rubella) Vaccine or Proof of Immunization
  4. Contractor Orientation
    • On Boarding: Ad-Hoc Orientation/Video by the Projects Office Team
    • Annual Contractor Orientation Every November (Mandatory)

All Contractors will report to the Project Management Office for document verification; once approved, the final badge authorization is conducted by PVHMC Security Department.

Once a security badge is issued, it is to be worn above your waist with photograph visible at all times.

Contract Worker Immunization Requirements Policy

The purpose of this policy is to establish a hospital policy for general and prime contract workers that provide services in or near patient care areas. This is to ensure the safety of our Associates, Contractors, Patients, and Visitors.

PROJECT MANAGEMENT OFFICE (PMO) IMMUNIZATION ON-BOARDING REQUIREMENTS

A. MEASLES, MUMPS, AND RUBELLA (MMR)

  1. Proof of immunity – Pomona Valley Hospital Medical Center (PVHMC) is required to know the status. Contractors must provide on of the following:
    • Written Documentation – Immunization Records (two doses given)
    • Titer Test (Lab results of immunity or confirmation of the disease)
  2. The PVHMC Project Management Office will not accept verbal reports of vaccination without written documents as proof of immunity.
  3. If proof is not available or negative titer – MMR Vaccination (RECOMMENDED)
  4. If a contractor declines the vaccination(s) recommended, once status is know, a declination statement must be signed and documented.
    • If declined, the contractor must wear a surgical mask in all patient care areas or working with associates.
    • All vaccinated contractors must wear a sticker provided by only the PVHMC Project Management Office.

B. INFLUENZA

  1. Pomona Valley Hospital Medical Center (PVHMC) recommends a seasonal influenza (flu) vaccine. Influenza season is from November 1st 2019 – April 30th, 2020.
  2. Compliance is mandatory through receipt of the vaccine or signing a declination form.
    • If declined, the contractor must wear a surgical mask upon entering the hospital.
    • All vaccinated associates and contractors must wear “current year” flu shot sticker distributed only by PVHMC Occupational Health Services (OHS) or the Project Management Office (Contractors only).

C. TUBERCULOSIS (TB)

  1. All Contract Workers that report to the PVHMC Project Management Office will be screened for TB annually.
  2. The contracting agency will be responsible for monitoring annual TB Screening compliance of workers assigned to PVHMC.
  3. TB Screening results will be provided by the contracting agency to the Director/Manager of the department/area of contract worker assignment before starting work.
  4. Contract workers may be referred to a Pomona Valley Health Center Clinic, Claremont or Chino Hills, for TB Screening if unable to obtain test through their agency. Applicable charges will apply.
  5. If TB Screening (TST – Mantoux or blood test) is positive, the clinic will refer the contract worker to the Agency’s health clinic for follow-up. Documentation of follow-up will be provided to the Industrial Clinic to obtain clearance to start work at PVHMC.

Immunization FAQ

Why am I required to share my immunization information with PVHMC as a contractor?

The County of Los Angeles Public Health (CDPH) issued a statement requiring all Hospital Administrators to develop a master list of the immunity status of all staff, vendors and contractors (not just healthcare workers). Due to this requirement, Pomona Valley Hospital Medical Center has taken the opportunity to review and revise our current vendor immunization requirements. All on-site and patient care Vendor Representatives as well as all Contractors will now follow an immunization standard that mirrors that of our employees and meets the CDPH and CDC recommendations.
